Social Services Director

Highbanks Care Center is currently seeking a Social Services Director to join our dynamic management team. The Social Services Director is responsible for managing and/or assisting with various resident services including the resident waiver program, PASSAR requirements, Level of Care (LOC), Care Planning, MDS completion as well as discharge coordination, care conferences and assisting with quality assurance for resident services.

The successful candidate will have previous experience in a long-term care environment and exhibit a compassionate and empathetic demeanor in caring for our residents. License to practice Social Work (LSW) in the State of Ohio required.
